We recognise that transitions can be difficult for a child with SEND and take steps to ensure that any transition is as smooth as possible.
If your child is moving to another school, we will contact the school SENCO and ensure he/she knows about any special arrangements or support that needs to be made for your child. We will make sure that all records about your child are passed on as soon as possible.
Before the end of each school year class teachers meet to pass on information about each child to the new class teacher. If necessary, a transition meeting can be arranged for you to meet and discuss your child’s needs with their teacher before the new academic year begins. Some children who are particularly anxious about transition will be given a transition booklet before the end of the year with photos of their new teacher, teaching assistant and classroom.
In Year 6 the SENCO and class teacher and in some cases the Learning Mentor will meet with the Primary Transition Team from your child’s new school and all information will be discussed and shared. Where appropriate, arrangements can be made for your child to visit their new school with a member of our staff to ease transition. The school will ensure that all records and assessments are discussed and shared with the new school.
